True Constipation (The Actual Condition)
True constipation, in Aajonus's framework, is a complete physical block in the intestines, a situation in which food and fecal matter literally cannot move through. He described it this way: "True constipation is when you have a block in your intestine and then peristalsis stops. It doesn't get beyond that point." Elsewhere he stated: "True constipation is when you've got a block in the intestines and nothing's moving through." And again: "Actual constipation is when you have a block in the intestines and the food is actually not moving through."
The physical presentation of true constipation, according to Aajonus, includes: the stomach swelling with everything you eat because nothing is passing, the sigmoid colon getting progressively larger and harder with accumulated fecal matter to the point where it itself becomes like a block, "of course, the sigmoid colon getting larger and larger with harder fecal matter is like a block", and in severe cases, the presence of a tumor or other tissue obstruction physically preventing the passage of fecal matter. He specifically called out the tumor scenario: "unless you've got a tumor blocking the passage, that's when it's a bad thing."
In the newsletter, he defined it clinically: "Constipation is the inability to defecate. That is caused by several differing circumstances in cases where no tumors or other tissue obstructions exist."
False Constipation, The Two Scenarios He Described
Aajonus outlined two entirely distinct scenarios that people wrongly label as constipation:
Scenario One: The Body Absorbs Everything
"The body digests and absorbs all of the food consumed and discards no waste." This is not constipation, this is exceptional digestion. He explained: "In the first case, constipation is the wrong term but that is what it is called. It usually occurs in people whose bodies are starved for nutrients."
He described the physical presentation: very small, very infrequent bowel movements, "a few inches, very skinny", or even no bowel movements for five to seven days at a stretch. People panic and think they are constipated, but according to Aajonus, they are simply so nutritionally depleted that the body is using absolutely everything they ingest. "If you digest everything, you're not going to have any byproducts. You're not going to have any waste." He added: "That's a miracle that you can digest that well that you're not having fecal matter. That's exceptional."
He documented this extensively in clinical practice: "I have known several people who had one or two small bowel movements weekly for the first 2-3 months on my PD." He had thousands of people he suspected were accumulating fecal matter, and sent them for ultrasounds, and found zero cases of fecal accumulation: "How many people out of the thousands have gone to have those ultrasounds come back with a high registration of fecal matter in their intestines? Zero. Not one have I ever found, except in the case of cancer."
He compared this phenomenon to wild animals: "Do you know why dogs and cats usually have fecal matter when they're eating their natural diet? Because they have to pass the fur that they can't digest. They digest everything and wouldn't have any feces if it weren't for the fur and some of the hide. If you ever examine a wild feces from a wild dog or a cat or wolf, you can't find anything in it but hair and some hide."
Scenario Two: Dry Fecal Matter Due to Fat Deficiency and Bacterial Deficit
The second form of false constipation is when fecal matter does form, but it becomes very dry, very hard, and very difficult to pass, sometimes large enough to cause rectal tearing. This is caused not by a physical block but by a lack of fat and/or insufficient E. coli bacteria reaching the colon and sigmoid colon to lubricate and process the fecal matter.
He described this as "non-lubrication at the end point", not a blockage but an absence of moisture and bacterial activity. He stated: "The other is just non-lubrication at the end point."
In the newsletter he listed the specific causes of this second scenario: "1. eating cooked food where everything has been fractionated into none bio-active couplings, 2. a fat deficiency from not eating enough fat, 3. eating concentrated mineral foods without enough fat, such as drinking milk that lacks enough cream or eating cheese alone, and 4. lacking proper levels of E. coli."
He described what happens in the sigmoid colon during this process in great mechanical detail: the body holds fecal matter in the sigmoid colon to extract every last bit of nutrient from it, particularly the waste products of bacterial digestion that are used to feed the brain and nervous system, and in doing so, all moisture and fat are sucked out of the fecal matter, leaving it hard as rock. "The longer the body is trying to pull all of the bacteria's waste product, which is our food, out of that fecal matter, so it gets rock hard and it gets big and sometimes it'll go into little balls and rip your rectum to pieces."
He emphasized this distinction clearly: "When your body's holding up fecal matter to get everything it needs out of poor quality food, it's going to be very dry. That's not true constipation. That's nutrient deficiencies. That's malnutrition."

Root Cause
Aajonus identified multiple interrelated causes of what people experience as constipation. He was most expansive and detailed about the role of E. coli, fat deficiency, and the destruction of intestinal bacteria through modern dietary and medical practices.
The Role of E. coli, The Central and Most Common Cause
Aajonus repeatedly and emphatically identified E. coli deficiency as the single most common cause of chronic constipation, particularly for people on the Primal Diet. He stated directly: "E. coli deficiency is the most common cause of chronic constipation on my PD."
He explained the entire physiological mechanism: E. coli are the dominant bacteria in the large intestine (the bowel). Their primary function is the final stage of digestion, breaking down fats and proteins into the microscopic, finite molecules small enough to feed the brain and nervous system. "E. coli is responsible for the final stage of protein digestion. It breaks those molecules of protein and fats down into the smallest finite substances to feed the brain and the nervous system."
He explained that the bowel, specifically the sigmoid colon, is where the body absorbs most of the nutrients intended for the brain and nervous system. "The bowel feeds the brain and nervous system. It's the last stage of digestion to get those molecules in the finite form to feed the brain and the nervous system. Because the brain and the nervous system have to have finite food molecules or really waste of bacteria."
When E. coli populations are depleted, several things happen simultaneously: (1) the brain and nervous system become undernourished, leading to depression, irritability, and psychological instability; (2) the sigmoid colon holds fecal matter longer and longer, trying to extract nutrients from increasingly barren material; (3) the fecal matter dries out completely and becomes hard, difficult, or impossible to pass; and (4) the entire lower digestive system ceases to function properly.
He stated: "A lot of people have constipation. Why do they have constipation? Because the E. coli is so low. Our food is so sterile. Packaged processed foods are so sterile. Our environment is so sterile that it kills the E. coli in the bowel."
He identified the required bacterial mass: "We should have 3 to 5 pounds of bacteria in our intestines." And regarding the composition of fecal matter: "60-80% of fecal matter is supposed to be bacteria. If we didn't have the bacteria, we'd have fecal matter the size of marbles. It would be the texture of granite and always rip us to pieces."
He noted the relationship between E. coli and the pharmaceutical industry directly: "So the pharmaceutical industry wants E. coli destroyed so your brain and nervous system don't function."
Causes of E. coli Deficiency
Aajonus detailed multiple pathways through which E. coli populations become depleted:
1. Long-term vegetarianism or fruitarianism: "Long-term vegetarianism or fruitarianism where the diet is too alkaline (too many fruits and vegetables) and not enough animal products. E. coli in the human body feed best on animal proteins." An overly alkaline intestinal environment, caused by too much fruit and vegetable consumption, destroys the acidic environment E. coli require to thrive and propagate. "E. coli require an acidic environment to thrive and propagate."
2. Toxicity in the bowel from chemicals, medications, and processed foods: "Toxicity in the bowel that poisons E. coli colonies (medications, pesticides, preservatives, additives and most chemicals, including chemicals formed by cooking)." He specifically noted: "When 1 & 2 above were combined, such as long-term vegetarians who ate lots of soy products, especially soy chips, recovering from chronic constipation while eating my PD can take 10-20 years."
3. Antibiotics and tetanus shots: Aajonus connected his own childhood constipation directly to the antibiotics and tetanus shots he received constantly as a child: "I had true constipation since I was a child that I can remember, all the antibiotics and tetanus shots. I was getting tetanus shots every three to six months. And so by the time I was probably eight years old, I had bowel movements once every three to five days."
4. Colonics and enemas: He was emphatic that colonics and enemas destroy E. coli populations and directly cause constipation. "Do you know anybody who takes regular colonics? How emotionally and psychologically stable are they? They don't have their colonic often. Very panicked. Very emotional. Overly emotional. So you don't wash out your E. coli. You eat better, so you're not constipated." He warned: "You don't take enemas which flush out your E. coli. It could take you 45, 60, even 90 days to replace that E. coli. That means you're going to probably be on the edge of depression, if not in depression" for the entirety of that recovery period. He also stated: "All kinds of things like that rips it apart" in reference to colonics.
5. Sterile modern environment: The overall sterility of processed foods, packaged foods, and the modern environment in general deprives the body of the constant replenishment of bacterial populations it evolved to receive. "Our food is so sterile. Packaged processed foods are so sterile. Our environment is so sterile that it kills the E. coli in the bowel."
Fat Deficiency as a Cause
Even with adequate E. coli populations, insufficient fat reaching the colon causes the fecal matter to dry out. He explained the mechanism: as food travels through the digestive system, fats are progressively absorbed by the organs and glands in the small intestines, by the time material reaches the sigmoid colon, very little fat remains. If the diet is low in fat, or even if it is adequate in fat but the body is so depleted that it absorbs everything before it reaches the lower bowel, the E. coli have nothing fresh to work with, the fecal matter is not lubricated, and it dries to a rock-hard consistency.
He stated: "A lot of people will get dry fecal matter if they're on a low-fat diet. Even if you've been on a very low-fat diet for many, many years and you go to a high-fat diet, you're still going to have dry fecal matter for a long time because your cells have been starving for fat for decades."
He described the mechanism in detail: "When food passes through the digestive tract, goes to the stomach, and then all of a sudden it's absorbed everywhere in the small intestines, by the time it gets to the ileocecal valve, going into the bowel, there's very little fat left. The bodies use it from the small intestines to feed the organs and glands, and nothing's left to feed the brain and the nervous system, which is what the bowel takes care of."
He gave a striking personal example: "I have people that eat sometimes a half a pound of butter a day and their skin is dry. Why? Because they're so deficient in fats on the glands and organs, there's nothing left to get to the skin." The same principle applies to the colon, even massive fat consumption may not be enough to saturate the starved tissues before fats are stripped out entirely before reaching the large intestine.
He explained that when the E. coli in the sigmoid colon are not receiving fresh fat from the diet, they are forced to continue breaking down the fecal matter that arrives to try to extract what little fat remains. This causes them to hold the fecal matter for increasingly long periods, drying it out progressively: "What's happening? They're not getting fresh food. They're not getting fresh, unadulterated nutrients, especially if they're eating a lot of cheese, they're absorbing the toxins, and that permeates the fecal matter. Then the poor E. coli is saying, listen, I've got to hold onto this."
Vegetables as a Cause of Constipation
Aajonus specifically identified vegetables, including raw vegetable consumption and vegetable juice with too much fiber, as a cause of constipation for people on a raw animal food diet. He explained the physiological incompatibility in detail:
"Vegetables are very difficult to digest in the human body. A vegetarian animal has a digestive tract two and a half times longer than ours. They have two to four stomachs, we have one. They have 60,000 times more enzymes to disassemble the cellulose molecule. It takes 48 hours even with them regurgitating, remasticating many times the same food to get that cellulose to break down. It takes 48 hours to pass through. It passes through us in 24 hours."
He noted that cellulose passing through the human system too quickly "secretes alkaline fluids", which destroys the acidic E. coli environment. He described a specific case: "I want to say this about constipation, vegetables on this diet will make you constipated." And he shared his own experience: "I did some vegetable juice last week with a juicer that wasn't my juicer. And it wasn't strained. And I had constipation for two days."
He described a patient named Marilyn, a health professional, who was eating salads every day while on the Primal Diet. Despite being told to stop, she continued eating salads because she believed fiber was necessary for bowel movements. He explained what was actually happening: "As long as she had the whole salad it was going to be filled with bacteria and with its feces, 80 to 90 percent bacteria, it acts like it builds and grows and grows and grows and thin like a sponge so that it takes that little toxic fiber toxic matter which would be that, like a rock in the system, and it inhalates it into a substance so it won't move through. If you have that bacteria destroyed, you'll end up with constipation. So she'd eat the salad every day and destroy that acid bacteria that would help blow up the feces. She had constipation."
After a year and three months, he convinced Marilyn to stop eating salads. Within two weeks, her constipation was gone completely.
He concluded: "It's not the fiber that does it. If you're on a quick diet, the fiber helps... So the diet of primarily toxic substances, it helps to move them through faster."
Cooked Food and Processed Food as a Cause
Cooked food contributes to constipation through multiple mechanisms: (1) It destroys bacterial populations in food that would normally replenish intestinal bacteria; (2) the cooking process fractures nutrients into non-bio-active forms that the body cannot use properly; (3) the chemical compounds created by cooking (lipid peroxides and other compounds) are toxic to E. coli; (4) fats in cooked food are absorbed by the small intestines but in cauterized, unusable forms that do not nourish the E. coli in the bowel. He stated: "Any good fats are going to be absorbed into small intestines and the colon won't get any."
He also noted that pasteurization at even 141 degrees for 15 seconds cauterizes 50% of calcium, making minerals harder and less pliable, which contributes to the hardening of fecal matter.
Insufficient Bacteria, The Mathematical Consequence
He returned repeatedly to the quantitative statement: without 3-5 pounds of bacteria in the intestines, and without 60-80% of fecal matter being bacteria, the physical character of fecal matter changes dramatically. The bacteria essentially act as padding, cushioning, and bulk, they swell and soften the fecal matter through their own mass and activity. Without them, fecal matter shrinks to marble-sized, granite-textured balls that tear tissue on exit.
Cheese Without Fat as a Cause
Aajonus identified eating cheese without adequate fat as a specific cause of constipation. Cheese acts as a sponge and magnetic absorber of toxins throughout the digestive tract, but this same absorptive quality, when exercised without fat present alongside it, dries out the digestive system. "It can dry the system, digestive tract. So, you need to eat an equal amount of butter with it or more. I'll eat sometimes one and a half to two times the amount of butter that I will cheese."
He gave direct dietary advice reflecting this: "If you get constipated from it, eat a little coconut cream with it or some butter. So you don't get constipated." And: "Eat cheese little amounts all day long... If you get constipated from it, eat a little coconut cream with it or some butter."
Raw Milk and Constipation
Interestingly, Aajonus also identified excessive raw milk consumption as a potential cause of constipation, because raw milk is so easily absorbed that the bacteria in the colon thrive excessively on it, consume all available fats, and leave nothing to lubricate the fecal matter: "A lot of people will experience constipation especially when they're drinking a lot of raw milk. It's easy to absorb. And guess what? The bacteria thrives so much on all that good food that everything dries. There's no fats left. So the fecal matter gets hard in some people. Very hard and hard to pass."

The destruction of E. coli by pharmaceuticals, antibiotics, colonics, sterile food environments, and alkalizing diets is the central mechanism causing chronic constipation. "The pharmaceutical industry wants E. coli destroyed so your brain and nervous system don't function."
Cooked Food: Cooking destroys the bacterial populations, cauterizes fats, denatures minerals, and creates toxic chemical compounds that poison E. coli, all of which contribute directly to constipation.
Raw Food / How to Eat: The solution to constipation, in almost every case Aajonus described, is a specific raw food approach, primarily raw fats delivered both orally and rectally, raw meat (especially high meat) to replenish bacterial populations, and specific combinations and avoidances.
Detoxification: The body holding fecal matter in the sigmoid colon is, in one sense, a detoxification behavior, the body attempting to extract as much usable material as possible from food before discarding it, because it is severely deficient. Constipation on the Primal Diet, in the false sense, is a detoxification marker indicating the depth of prior nutritional deprivation.
Sovereignty: Aajonus framed the medical establishment's war on E. coli as a deliberate strategy to keep people neurologically impaired and dependent. "So the pharmaceutical industry wants E. coli destroyed so your brain and nervous system don't function. So you know their little ploy."

Symptoms Reframed
Infrequent bowel movements (every 5-7 days) = Exceptional digestion, not constipation
The most fundamental reframe. If a person is not moving every day, or even every few days, Aajonus's first question was whether actual fecal matter was accumulating. If it was not, if the person simply felt no urgency and produced little output, he identified this as the body absorbing everything it was given. "You're not getting constipated. Your body is absorbing everything, absolutely everything you eat. That's a miracle that you can digest that well."
He documented his own experience: "I used to only go to the restroom once every five to seven days. Literally once every five to seven days." He also stated: "I have known several people who had one or two small bowel movements weekly for the first 2-3 months on my PD. I experienced rare bowel movements for weeks after fasting."
He noted that the medical profession's insistence on daily bowel movements is a construct: "The medical profession has drummed it into your brain. You have to go to the toilet every day."
Hard, brick-like fecal matter = Fat deficiency and E. coli deficiency, not a disease
When fecal matter does form but is rock-hard, marble-sized, and causes tearing on exit, this is not a disease requiring laxatives. It is a sign that the E. coli in the sigmoid colon are not receiving adequate fresh fat, and are therefore extracting all moisture from whatever fecal matter arrives. The solution is to deliver fat directly to the colon via suppository, not to take laxatives that destroy intestinal bacteria.
Small, infrequent, skinny fecal matter = Miracle digestion, not disease
"You're going to have very little fecal matter. A few inches, very skinny. Oh my god, I'm getting constipated. No, you're not getting constipated." This is the body utilizing nutrients at maximum efficiency.
Fecal matter every 5-7 days but large and hard when it comes = Two different phenomena in one event
This is the body absorbing everything it can (scenario one), but without adequate fat and bacteria in the sigmoid colon, what little fecal matter does form becomes desiccated and hard (scenario two). Aajonus experienced this himself for decades: "Some people will go 5 to 7 days without a bowel movement. But when it comes out, it's like a brick. Because the body's just sucking everything it can out of it. Digesting and sucking everything it can. That's not constipation. That's incredible, happy digestion."
Brain fog, depression, irritability = Signs of E. coli deficiency manifesting as constipation
When E. coli populations are insufficient to feed the brain and nervous system via the sigmoid colon's absorptive function, the psychological and neurological consequences manifest before the physical constipation becomes severe. He noted: "You notice people with colon problems have more of a problem with positivity and happiness. They are more irritable." And: "All those people who do enemas, you notice those people who do enemas and colonics become more sensitive every year. They become more emotional."
He specifically noted that delivering fat to the colon via suppository could resolve depression "in minutes." This is not metaphorical, he was describing the direct neurological feeding that occurs when E. coli receive fresh fat and produce the finite molecules that enter the brain and nervous system through the sigmoid colon's absorptive surface.
"This diet constipates me" = The diet is working too well
He directly addressed this common complaint from new people on the Primal Diet: "A lot of people get on this diet and they stop having malnourishment. But this diet constipates me. No, it doesn't constipate you. You're digesting every particle. You don't have any waste."
Tan, ropy feces with white blobs = Detoxification in progress
From a direct Q&A: one patient described feces that were "tan, ropy, and have strange white blobs attached", coming out in small pieces or long strings, with the anus muscles contracting shut, preventing elimination. Aajonus responded by recommending the butter/coconut cream/raw milk suppository formula and specific positioning techniques.

Aajonus developed an extensive, layered protocol for addressing constipation, ranging from dietary adjustments to mechanical interventions to parasite-based therapies. He organized these from least to most aggressive.

This was Aajonus's most consistently recommended intervention for constipation. He described it as the solution he "worked out about three years ago" (from the time of the seminar). He stated: "I got it in there, you have to understand what constipation is", and then proceeded to describe the formula in detail.
Standard Adult Suppository Formula:
- 3 tablespoons raw, no-salt butter
- 3 tablespoons dairy cream (raw)
- 3 tablespoons coconut cream
- ¼ teaspoon honey
Preparation: "Put that in a four-ounce jelly jar and you shake it. Then you immerse it in a bowl of hot water." The hot water warms the formula to body temperature so it can be drawn into a bulb syringe. He specified: "A four-ounce bowl, eight-ounce if you don't have a four-ounce bulb syringe, you suck that, you shake it up, and then you suck it up into the bulb syringe."
Administration: "Get on all fours, bark like a dog", he meant literally position on hands and knees. Insert the syringe into the rectum and squeeze the formula up into the sigmoid colon. Then immediately move to the next position: "Go down on your elbows so you're like this" (elbows on the floor, buttocks elevated) "and roll your stomach like a belly dancer. Breathe heavily to get it to move into this whole descending colon area so get up in the sigmoid colon."
He also described rolling to one side: "Roll on your left side. Lift your right leg in the air. And roll your stomach. And that will get it to move up the descending colon. Then you do that for only three minutes. Then you roll on your right side. Lift your left leg in the air. And roll your stomach again. And that will get it to move over the transverse colon down the ascending colon."
In another description he said: "Get on all four legs. You bark like a dog. And you shove it up in there, and you're going to feed it. You're not going to enema. You're going to feed the bacteria."
Timing: "Do this before you go to sleep at night or whenever your sleep period is, long sleep period." He said to do it the night before sleep so the formula can work overnight. "You leave it there. You go to sleep."
Frequency: "Every two to five days depending upon your constipation levels." He described a progression: "It could be every two days that you have to do it at first. After a couple of weeks, then every three to four days. Then every week. And then eventually when you've got such a population and you're eating very good and always supplying good fats, enough to get to the bowel, you'll never have constipation again."
Mechanism: The coconut cream portion is important because the body cannot digest coconut fat with the same bile-driven process it uses for dairy fat, coconut fat passes more intact into the colon, where it remains to soften the fecal matter. He explained: "You can still rob all the good animal fats out of it, but leave some coconut cream in there to soften the fecal matter. So you're feeding the bacteria the butter and dairy cream right from the back end. All those bacteria are very happy getting everything they need."
He also said: "The fat will be broken down into particles that will go to the brain and nervous system over the next two days."
Emotional and Neurological Effects: He described the experience: "Let me tell you, they rejoice. They let you have your poop. And they get strong. And you'll see the whole area around this bowel that's normally dry and bad in color and circulation change. The whole area will change." And: "Depression. A lot of psychological problems will disappear in minutes. If you do that." He further stated: "One out of probably I've gotten 600 people to do it and only one said he had no effect one way or the other. But he wasn't depressed anyway."
For babies: For an infant with constipation, he recommended: "Give him a suppository of 1 T. each of no-salt raw butter, coconut cream and dairy cream. That way the intestinal bacteria won't have to hold the feces so long to get as much fat as possible."
Variation with kefir: "Some people even take kefir yogurt and enter that, you know, use a suppository to help break down the elements in your bowel to a smaller degree."
In another Q&A context for a patient with both vaginal and rectal pain: He suggested: "Implanting a mixture of 3 tablespoons each of no-salt raw butter, coconut cream and raw milk into both your vagina and rectum, half of the formula in each orifice. Make certain you are lying down on your back for this. Once you have injected the formula in each orifice, raise your buttocks in the air, feet and upper back on the floor or bed, and slowly roll your stomach like a belly dancer for about 4 minutes."

Aajonus described what he called his "instant bowel movement constipation formula", a combination of foods that deliberately do not digest together, forcing rapid transit through the intestines. He was clear: this is not for regular use and does not provide nutrition. It is strictly for people who are truly blocked and need immediate relief.

He explained: "None of that digests. So it just pushes right through. You won't digest anything from it. You may flush some nutrients through but you usually have a bowel movement within 24 hours or less." He noted: "Some people, you know, who had to have bricks and pry it out with a crowbar or something, it's taken two times in two days, but that's rare."
He explained why it works: milk is acid, apple cider vinegar is acid, egg is acid, but the combination of milk and vinegar together prevents proper digestion, and the body moves this through the intestines faster because it cannot process it. He noted that cream and juice is an exception in his system because cream is not digested by hydrochloric acid but by the liver, and specifically mentioned "that's only for people who are constipated."
He placed this formula in context: "This is not suggested for regular use", he said it in the recipe book context as an "aggressive remedy" because it prevents proper digestion.

He had suffered from constipation since approximately 18 months of age, going every three to five days, with fecal matter "bigger than my fist and just as rough as granite," ripping his rectum every time until it was "90% scar tissue" by age 17 and "an inch and a half scar tissue thick all the way around" by age 20.
Despite going on a raw diet at age 21-22, he continued to have constipation until he accidentally acquired a tapeworm while eating raw meat in Asia: "About six weeks later, my bowel movements got better and better."
